In addition to behavioral health care, Metrocare provides primary care centers for adults and children, services for veterans and their families, accessible pharmacies, housing, and supportive social services. Alongside clinical care, researchers and teachers from Metrocare’s Altshuler Center for Education & Research are advancing mental health beyond Dallas County while providing critical workforce to the state.**Job Description:**The H.O.P.E. (Helping Others Progressively Evolve) Program Case Manager provides intensive, participant-centered case management for adults involved in the criminal justice system who have behavioral health, substance use, and co-occurring needs. The Case Manager serves as the primary point of contact for Dallas County Criminal Justice participants enrolled in the H.O.P.E. Program.The Case Manager conducts comprehensive assessments, develops individualized service plans, coordinates access to behavioral health and community-based services, and builds collaborative treatment teams that include participants, family and natural supports, behavioral health providers, probation officers, attorneys, peer recovery specialists, and community partners. The Case Manager promotes participant engagement, accountability, recovery, and successful community reintegration through coordinated, evidence-based case management.This position requires a professional who can build trusting relationships while maintaining clear professional boundaries and supporting the expectations of a specialty program. Using knowledge of Social Drivers of Health, Trauma-Informed Care, Recovery-Oriented Systems of Care, Harm Reduction principles, and Motivational Interviewing, the Case Manager coordinates services that address participants' behavioral health, substance use, housing, employment, education, transportation, healthcare, recovery support, and other social service needs while promoting long-term stability and reducing recidivism.**RESPONSIBILITIES:*** Serve as the primary point of contact for H.O.P.E. Program participants, providing ongoing support, advocacy, engagement, and care coordination throughout program participation.* Conduct comprehensive behavioral health, psychosocial, and resource assessments to identify participant strengths, needs, barriers, and recovery goals.* Develop, implement, monitor, and update individualized, person-centered service and recovery plans that support treatment participation and successful completion of program requirements.* Utilize Motivational Interviewing, strengths-based practices, trauma-informed care, and crisis intervention techniques to promote engagement, accountability, behavior change, and recovery.* Maintain regular contact with participants through office visits, telephone contacts, community visits, and collateral contacts as appropriate.* Coordinate behavioral health treatment, substance use treatment, psychiatric services, medical care, medication management, peer recovery support, and other community-based services.* Facilitate referrals, coordinate appointments, monitor attendance, and follow up to ensure successful linkage with recommended services.* Coordinate continuity of care during transitions between incarceration, residential treatment, outpatient treatment, hospitalization, community supervision, and recovery support services.* Assist participants in addressing Social Drivers of Health, including:* Housing* Food security* Transportation* Employment* Education* Government identification* Health insurance* Public benefits* Financial assistance* Recovery support services* Support participants in developing independent living skills, problem-solving abilities, relapse prevention plans, healthy coping skills, and natural support systems.* Monitor participant compliance with treatment recommendations, psychiatric appointments, medication management, counseling sessions, recovery meetings, and program requirements.* Attend multidisciplinary staffing meetings and specialty program sessions.* Provide timely verbal and written updates regarding participant progress, treatment engagement, compliance, barriers, behavioral concerns, and recommendations.* Collaborate with judges, attorneys, probation officers, treatment providers, psychiatrists, peer specialists, housing providers, employers, educational institutions, and community partners to promote participant success.* Assist the multidisciplinary team in identifying appropriate incentives, therapeutic interventions, and graduated responses to participant behavior.* Coordinate transition planning and community reintegration services for participants entering or leaving residential treatment, incarceration, hospitalization, or other levels of care.* Develop and maintain collaborative relationships with behavioral health providers, substance use treatment programs, hospitals, housing agencies, educational institutions, employers, and community organizations.* Identify service gaps and advocate for resources that improve participant outcomes and continuity of care.* Maintain accurate, objective, and timely documentation of assessments, participant contacts, referrals, service plans, treatment progress, outcomes, and program-related activities within the electronic health record.* Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, agency policies, and program disorders, recovery support services, Social Drivers of Health, and community resources.* Knowledge and application of Trauma-Informed Care, Harm Reduction, Motivational Interviewing, Recovery-Oriented Systems of Care, and Person-Centered Planning.* Strong verbal, written, and interpersonal communication skills with the ability to build rapport, maintain professional boundaries, and effectively engage justice-involved individuals.* Ability to collaborate effectively with multidisciplinary teams, criminal justice partners, treatment providers, and community agencies.* Strong advocacy skills with the ability to identify barriers, coordinate services, and promote recovery-oriented outcomes.* Demonstrates cultural humility, adaptability, resilience, professionalism, and respect for diverse populations.* Maintains accurate documentation and confidentiality in accordance with HIPAA, 42 CFR Part 2, agency policies, and program requirements.**QUALIFICATIONS:****Required Education, Experience, Licenses, and Certifications*** Bachelor's Degree from an accredited college or university with a minimum of 30 credit hours in a social, behavioral, or human services field and two (2) years of experience providing case management, behavioral health services, substance use treatment services, or criminal justice services.* Acceptable coursework includes psychology, social work, counseling, sociology, rehabilitation, nursing, medicine, human growth and development, educational psychology, gerontology, physician assistant studies, early childhood intervention, and related behavioral sciences.* Valid Texas Driver License.* Reliable transportation.* TB Test, CPR, First Aid, and SAMA completed within 30 days of hire.* TAC web-based training.* Certified Community Behavioral Health Clinic (CCBHC) trainings completed within the first year of employment.**Preferred Education, Experience, Licenses, and Certifications*** Master's Degree from an accredited college or university in Social Work, Counseling, Psychology, Behavioral Health, Criminal Justice, Public Health, or another related behavioral science field.* Experience working with:* Adult Specialty
