The Family Nurse Practitioner (FNP) provides primary care to a diverse population of homeless and low-income patients across the lifespan, in a Federally Qualified Health Center. The FNP provides equitable, team-based integrated and culturally responsive patient-centered care. The FNP diagnoses, treats and manages acute and chronic health conditions, and provides preventive care focused on health risk factor reduction and patient self-management. The FNP works in a care team environment and ensures efficient, effective, and integrated patient care through joint planning and problem solving with internal and external care team members.

Required:
• OR RN license (NURSYS)
• OR NP license
• NPI number

• DEA registration
• OPDMP (Oregon Prescription Drug Monitoring Program) registration is also required.
