Float Physical Therapist (PT) | Full-Time | Outpatient | Roanoke, VA Region

H2 Health is seeking a Float Physical Therapist to support outpatient clinics throughout the Roanoke, Virginia market, including locations within the H2 Health family such as H2 Health, Lucas Therapies, and SORVA Rehabilitation & Wellness. As a Float Physical Therapist, you'll have the opportunity to work alongside multiple experienced clinical teams while treating a diverse outpatient population. You'll provide individualized, evidence-based care to patients recovering from orthopedic injuries, surgery, balance and vestibular disorders, neurological conditions, musculoskeletal impairments, and mobility limitations. This role offers an ideal blend of clinical variety, professional development, and community impact while allowing you to build relationships across several highly respected outpatient clinics.

Your Role:

• Perform comprehensive patient evaluations and develop individualized plans of care.
• Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
• Treat a variety of orthopedic, post-operative, neurological, vestibular, balance, musculoskeletal, and mobility-related conditions.
• Provide exceptional patient care across multiple outpatient clinic locations throughout the Roanoke region.
• Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
• Educate patients on injury prevention, wellness, and home exercise programs.
• Maintain accurate and timely documentation of patient progress and treatment outcomes.
• Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
• Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
• Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
• Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.

Requirements

• Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
• Current Virginia Physical Therapist license or eligibility for licensure.
• CPR certification or willingness to obtain as required.
• Ability and willingness to travel between assigned clinic locations.
