As a RN MDS Nurse at Hudson Springs Nursing and Rehab, you will play a vital role in delivering high-quality, patient-centered care to our residents. Our mission is to help individuals return home as soon as possible, and we're seeking a qualified and compassionate nurse to join our team.

About the Role:

• As an RN MDS Nurse, you will be responsible for developing, implementing, and evaluating individualized care plans for our residents, ensuring that their physical, emotional, and social needs are met.

• You will work closely with our interdisciplinary team to coordinate care, manage patient flow, and ensure that our residents receive the best possible outcomes.

• You will also be responsible for conducting MDS assessments, developing care plans, and communicating with residents, families, and healthcare providers.

• Our newly constructed center offers a spa-like environment, two leading-edge therapy gyms, and advanced therapies to ensure top-quality transitional care.

Responsibilities:

• Develop, implement, and evaluate individualized care plans for residents

• Conduct MDS assessments and develop care plans

• Coordinate care with the interdisciplinary team

• Manage patient flow and ensure efficient care delivery

• Communicate with residents, families, and healthcare providers

• Participate in Quality Improvement initiatives

• Adhere to regulatory requirements and company policies
