RPM Care Manager - On-Site Patient Engagement Pro

EPIC Health in Michigan is seeking a compassionate RPM Care Manager to support patients enrolled in our Remote Patient Monitoring program. This on-site role focuses on monitoring readings, engaging patients, educating them, and coordinating timely clinical follow-up.
