Field Case Manager - RN - Dallas, TX

Primary Responsibilities: Assess, plan, and implement person-centered strategies Connect members with financial, psychosocial, community, and state resources Coordinate care across the continuum and serve as a key point of contact Advocate for members and families to ensure their needs and preferences are heard and supported Collaborate with providers, members, caregivers, and community partners to improve health outcomes Travel up to 75% of the time within the Mission, TX area and surrounding areas You will be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Current unrestricted Texas RN license 2+ years of experience in a community health or healthcare setting Intermediate level of proficiency with Microsoft Office applications High-speed internet at residence Access to reliable transportation and valid US driver's license with the ability to travel within assigned territory Preferred Qualifications: Experience with Medicaid, LTSS, or long-term care services Knowledge of HCBS and integrated care principles Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations Bilingual in English and Spanish *All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.
