Job Title: Home Health Physical Therapist Location: Community-Based (Patient Homes & Local Service Area) Chico, Oroville, Marysville Employment Type: Full-Time / Contract Position Summary The Home Health Physical Therapist provides skilled physical therapy services to patients in their homes, focusing on restoring function, improving mobility, relieving pain, and preventing or limiting permanent physical disabilities. This role emphasizes personalized, patient-centered care while supporting individuals in maintaining independence within their community. Key Responsibilities Perform comprehensive physical therapy evaluations in patients’ homes to assess mobility, strength, balance, coordination, and functional abilities Develop and implement individualized treatment plans based on patient needs, physician orders, and clinical findings Provide therapeutic exercises, manual therapy, gait training, and balance interventions Educate patients, families, and caregivers on home exercise programs, safety strategies, and fall prevention Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes Document all visits, evaluations, and progress notes accurately and in compliance with regulatory and agency standards Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care Assess home environments and recommend modifications or adaptive equipment to improve safety and independence Participate in case conferences and interdisciplinary team meetings Qualifications Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program Current state licensure as a Physical Therapist Minimum of 1 year of clinical experience (home health experience preferred) Valid driver’s license and reliable transportation Current CPR certification Skills & Competencies Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Ability to work independently and manage time effectively Compassionate, patient-focused approach to care Knowledge of home health regulations and documentation standards Adaptability to diverse home environments and patient conditions Work Environment Travel to patient homes within the assigned service area Flexible scheduling based on patient needs Exposure to various home conditions and environments Benefits (if Applicable) Competitive salary and mileage reimbursement Flexible scheduling Health, dental, and vision insurance Paid time off and holidays Continuing education opportunities #J-18808-Ljbffr Talented Medical Solutions
