Physical Therapist - Home Health

Position Overview: We are seeking a compassionate Home Health Physical Therapist to evaluate, develop, and implement individualized care plans for patients in their homes. This autonomous role focuses on helping patients recover mobility, strength, and independence through hands-on therapy, education, and collaboration with interdisciplinary teams. Key Responsibilities Assess patients' functional status, mobility, and rehabilitation potential Create and adjust personalized care plans in coordination with physicians and care teams Provide hands-on therapy to improve mobility, balance, and strength Educate patients and caregivers on exercises, safety, and treatment strategies Monitor progress, support safe care transitions, and participate in discharge planning Collaborate with nurses, therapists, and physicians to ensure coordinated care Document care and progress accurately in EMR systems Supervise Physical Therapist Assistants and support caregiver education Qualifications & Skills Graduate of an accredited Physical Therapy program (DPT or Master’s) Current PT license in the practice state 1–2 years of experience, preferably in home health Strong clinical assessment, critical thinking, and communication skills CPR certification, valid driver’s license, reliable transportation, and auto insurance Ideal Candidate Traits: Autonomy, passion for patient independence, relationship-building skills, clinical excellence, and compassionate care. #J-18808-Ljbffr
