Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing
• Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations

Required Qualifications

• Active Physical Therapist licensure in good standing in the applicable state
• Minimum of 1 year of outpatient physical therapy experience
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Knowledge of and adherence to discipline-specific professional code of conduct and state practice act requirements
• Proficiency in clinical documentation, including compliance with state, federal, and payer guidelines

Preferred Experience / Skills

• Experience supervising or mentoring physical therapy students in a clinical setting
• Familiarity with performance improvement methodologies and quality initiatives in a rehabilitation setting
• Experience working within insurance authorization processes and physician order requirements
