Segue Health, a faith-based medical practice in Forney, TX, seeks an experienced Nurse Practitioner to provide in-home post-discharge care as patients transition from hospital to home. You will conduct in-home visits, review discharge summaries, and coordinate with home health professionals to ensure seamless care.
