What Will You do as a Community Paramedic with IHCare+ IHCare+ paramedics provide scheduled, non‑emergent home visits focused on assessment, early intervention, and patient coaching rather than traditional transport‑driven EMS response. Typical activities include head‑to‑toe assessments, medication review, vitals and diagnostic checks, chronic disease monitoring, and escalation or teleconsultation with partnering clinicians when needed. Our Community Paramedics also help identify and address barriers like transportation, health literacy, and access to primary care or behavioral health services, connecting patients with community resources and support programs. Documentation is completed in an electronic health record, supporting data‑driven quality improvement and program outcomes tracking. The Community Paramedic is responsible for providing in‑home and mobile healthcare services to underserved and under‑resourced populations. This role includes conducting health assessments, delivering care, offering chronic disease management, assisting with care coordination, and addressing social determinants of health. The Community Paramedic will collaborate with healthcare providers, Managed Care Organizations (MCOs), and social service agencies to support improved health outcomes and reduce hospital readmissions.
