The incumbent will serve as a high level professional whose activities will span the interface between the physicians and primary nurses who manage inpatients on the Liver Transplant Services at Moffitt-Long Medical Center, UCSF. The Nurse Practitioner will act as a full member of the Liver Transplant Team, taking primary responsibility for the safe delivery of care to inpatients in consultation with attending's or residents. This team member will meet productivity expectations as set forth by the Chief of Transplantation, performing any duties related to patient care activities that are appropriate and clearly delineated. The incumbent will be responsible for the inpatient care of patients admitted to the liver transplant service. The incumbent must have excellent communication skills and be able to work with a multidisciplinary team that includes attending surgeons and physicians, resident physicians, medical students, pharmacists, and social workers. The person in this position complements and assists the work of the surgical residents and the attending staff. All activities will be performed as a part of the physician team and under direct or indirect supervision. Under the supervision of the UCSF Department of Transplant Surgery and guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique ambulatory patient services this Nurse Practitioner (NP) position is assigned to in Liver Transplant. This AHP position may perform delegated functions as appropriate to their "scope of practice". In general, the "scope of practice" includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per approved standardized procedures as identified by the list available on the Medical Staff Office website. The CIDP approved standardized procedures are unique to each person and are unlikely to include ALL SPs on the list. This position may provide patient care in approved age groups and UCSF Health licensed settings. The AHP has responsibility for the quality of standards of care for a specified patient population and evidences self‑direction and accountability in the development of this role within the all UCSF medical center locations. UCSF Health requires all Advanced Health Practitioners (AHP) to be credentialed through its Medical Staff Services Office (MSSO) in order to practice and function in their clinical role. This applies to both adult and pediatric AHPs in the inpatient and outpatient clinical settings at all UCSF Health sites (Parnassus, Mt. Zion, Satellite Clinics, et al). Credentialing and approval of the Standardized Procedures takes between 30‑60 days and must be completed prior to the first working day. The APP Will: Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Evaluate for and order home health services, durable medical equipment (DME), Department of Motor Vehicles (DMV) certifications, refer for consultations, physical therapy, order radiographic imaging or diagnostic tests, and specialized treatments (e.g. chemotherapy with the approved privilege). Provide patient and family education, arrange follow-up appointments, and engage in any approved privileges after the APP has demonstrated competency through proctoring and FPPE process. OMAG will identify independent practice for approved privileges on the ECHO website and procedural activity will be entered by the provider and monitored via the elog system To evaluate, assess and create a documented plan of treatment per patient's presenting HPI or established plan of care as defined by the UCSF Medical Staff Bylaws. To answer clinically relevant patient medical advice calls or during bedside communications or in coordination with health care professionals, patients, and their families as necessary. To independently, or in a shared visit model, evaluate routine, acute, critical, new patients, or specialty consultations when needed or required and discuss with attending and staff as appropriate and document in the EHR. Any required documentation workflow must be adhered to. Be able to interpret and act upon ordered tests, procedures, and radiographic studies. To assist in formulating and implementing protocols to improve efficiency in the clinical specialty Effectively communicate with family or patients in person, via telephone and/or via other electronic modalities in a timely manner. Ensure in their absence, appropriate clinical coverage has been arranged for patient management and communication as appropriate. At all times adhere to all policies and procedures of UCSF Health in terms of HIPPA/Communication/Privacy/Code of Conduct, and any other UCSF or Medical Center Policies, which includes meeting the encounter closure requirements of the Medical Staff Bylaws Mandatory attendance at APP UCSF Onboarding Sessions for all new APP employees whether experienced or new to APP practice. Adhere to all scheduling and communication policies whether specific or as outlined in the collective bargaining agreement. Some duties, performed routinely or periodically, may be unique to the specific clinical service of the APP Function as appropriate to their "scope of practice." In general, the "scope of practice" includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per their approved standardized procedures. Familiarity with state statute for their licenses is necessary. FOR INPATIENT APPS: Assist in formulating and implementing protocols to improve efficiency in the clinical specialty/service/pathways. Engage in a timely and efficient discharge of patients on the inpatient service for which you are responsible. Assist in coordinating patient transfers within and outside UCSF Health according to policy. Be familiar with outcome metrics, guidelines, and expectations for providers in clinical specialty.
