Promotes effective working relations and works as part of a team to meet departmental goals and objectives. + Maintains current knowledge, certification, and licensure, and pursues ongoing professional growth and development. + Provides patient education and support to address identified needs. + Monitors and evaluates patient progress and adjusts care plans as necessary. Collaborates with other healthcare professionals to ensure comprehensive patient care. + Participates in quality improvement initiatives to enhance patient care. + Other duties as assigned. **Knowledge, Skills, and Abilities:** + Handles multiple priorities.
