License: Current RN licensure permitting work in the state; Current Basic Life Support (BLS) certification required

Certifications: Advanced life support certification (ACLS and PALS) may be required for some departments

Must-Have: EPIC electronic medical record experience required; No large gaps in the work history during the 13-week assignment

Description: The candidate will be responsible for all aspects of the transitional plan of care from admission to discharge. Comfort with conducting the initial case management assessment interview is necessary. The candidate will provide transitional care follow-up as needed. Ensuring safe discharge to the patient's final destination, whether home, self-care, home health, skilled nursing facility, inpatient rehab, etc., is essential. Candidates should be highly skilled and able to multitask, prioritize, document accurately, and collaborate with community partners and vendors. Knowledge of Medicare, Medicaid, and insurance coverage is beneficial but not required. Understanding the basic rules and regulations governing acute inpatient case management will be helpful. Providing mental health support/therapy will not be part of the role; specialized teams will handle this. Candidates will work collaboratively with behavioral health facilities and specialists, especially when managing a high population of behavioral health cases. Knowledge of appeals and denials, discharge planning, and HIPAA guidelines is preferred. Ability to coordinate care and assess needs is vital. Experience in managing patient care pathways and authorizations is necessary. The candidate will float rarely to a sister location when needed.
