Education & Certification Requirements:

• Graduate of an accredited Nursing program (LPN or RN).
• Active South Carolina nursing license in good standing required.
• Current CPR/BLS certification required.
• Strong commitment to patient-centered care, clinical excellence, and ongoing professional development.
• Proficiency with electronic medical records (EMR/EHR) systems and clinical documentation.

Experience Requirements:

• Minimum of 2 years of nursing experience in a physician practice, family medicine, primary care, specialty clinic, urgent care, community health center, or other outpatient healthcare setting.
• Experience with patient triage, care coordination, medication administration, immunizations, chronic disease management, and patient education.
• Ability to work independently while collaborating effectively with providers, clinical staff, and patients in a fast-paced medical office environment.
• Strong clinical assessment, critical thinking, communication, and organizational skills.
• Bilingual English/Spanish skills highly valued.
• Passion for building patient relationships and delivering exceptional customer service.

Job Duties:

• Serve as a key member of the healthcare team by delivering high-quality nursing care and supporting positive patient outcomes.
• Perform patient assessments, clinical triage, treatments, injections, immunizations, medication administration, and follow-up care.
• Educate patients on preventive health, wellness programs, chronic disease management, medications, and treatment plans.
• Coordinate care through referrals, prior authorizations, prescription refills, diagnostic testing, and follow-up communications.
• Monitor and communicate patient conditions, concerns, and treatment responses to providers.
• Maintain accurate, timely, and compliant documentation within the EMR system.
• Collaborate with physicians and healthcare staff to improve patient access, workflow efficiency, and overall patient satisfaction.
• Deliver compassionate, professional care while creating a welcoming and supportive experience for every patient.
