Responsibilities
- Perform comprehensive OT evaluations for adults in acute care and develop individualized treatment plans to address ADLs, mobility, cognition, and coordination.
- Implement evidence-based OT interventions to improve functional independence and quality of life.
- Facilitate discharge planning and transition to subsequent care settings, coordinating with the multidisciplinary team.
- Document patient progress, goals, and outcomes in the appropriate electronic medical record system; maintain accurate and timely records.
- Educate patients, families, and care teams on therapy goals, safety, and home programs; participate in care rounds and team conferences.
- Demonstrate professional collaboration, adaptability, and adherence to state licensing and facility standards.

Requirements
- Active Minnesota Occupational Therapy license or eligibility for licensure
- Degree in Occupational Therapy from an ACOTE-accredited program
- Ability to work in an acute care rehabilitation environment
- Willingness to fulfill a travel assignment with 40 hours per week
- Strong assessment, documentation, and collaboration skills
- Excellent communication with patients, families, and the interprofessional team
