Tryfacta is seeking a Licensed Practical Nurse (LPN) – Long-Term Care (LTC) for our client in Westbrook, ME 04092. This is a 14-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Location: Westbrook, ME 04092

Duration: 14-week

Work Schedule: Evenings (2:45 PM – 11:45 PM) Guaranteed Hours: 40 Hours/Week

Responsibilities for this position include, but are not limited to:

• Administer medications and treatments as prescribed by the physician
• Monitor and assess residents' health conditions and report changes to the RN or physician
• Provide direct nursing care, including wound care, catheter care, and other clinical procedures
• Maintain accurate and timely patient documentation
• Assist residents with activities of daily living (ADLs) as needed
• Collaborate with physicians, RNs, CNAs, and other healthcare professionals to ensure quality patient care
• Follow infection control policies, safety standards, and facility guidelines
• Deliver compassionate, resident-centered care while promoting dignity, comfort, and independence

To be considered for this position, you should have:

• Minimum 0.5 – 1 years of Long-Term Care (LTC) experience preferred
• Experience in Skilled Nursing Facility (SNF) settings preferred
• Strong medication administration and documentation skills
• Excellent communication and patient care abilities
• Ability to work independently and as part of a multidisciplinary team
• Graduate of an accredited Practical Nursing (LPN) program
• Active Vermont LPN License (or Compact License, if applicable)
• BLS Certification required
