Work Environment

This is an onsite position primarily set in the home care environment, where our team members visit patients at their residences, providing personalized care tailored to their unique needs. Key Responsibilities

• Conduct comprehensive evaluations, including a detailed review of each patient's medical history, performing specific assessments to design tailored treatment plans.
• Communicate evaluation findings and treatment recommendations clearly to patients and their families, ensuring active involvement in goal setting.
• Implement treatment protocols proficiently based on evaluation results, the patient's presentation, and advancements in healthcare practices.
• Collaborate with other healthcare professionals to modify treatment plans as necessary based on changes in a patient's condition, ensuring optimal patient care.
• Perform timely discharge planning and follow-up care, meticulously documenting all patient interactions and treatment activities to maintain accurate patient records.
• Demonstrate effective time management and meet expected productivity standards while maintaining an accurate schedule and being punctual for meetings.
• Contribute actively to departmental goals and objectives and comply with relevant governmental and accreditation regulations.

Patient Population / Client Focus

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Current and valid state licensure to practice as a Physical Therapist in New Jersey.

Preferred Experience
