Advanced Practice NP/PA: Geriatric Care & Home Visits

Element Care Inc. is seeking an Advanced Practice Provider (APP) – NP/PA to join the PACE interdisciplinary team, delivering comprehensive primary care in various settings and collaborating with Medical Doctors to meet care plan objectives. The APP will conduct annual and semi-annual exams, evaluate participants across care settings, perform home visits as needed, participate in after-hours on-call coverage, and provide education to patients and families while documenting thoroughly and adhering #J-18808-Ljbffr Element Care Inc.
