Work Environment

This position operates onsite, primarily in clients' homes throughout Newark and the surrounding areas. As a Physical Therapist within a home care setting, you will have the opportunity to create a warm and supportive environment for patients of varying ages and health conditions. Key Responsibilities

• Develop and oversee individualized physical therapy treatment plans aimed at preventing disability, relieving pain, and enhancing muscle function based on the patient's unique needs.
• Conduct thorough evaluations of clients' physical capabilities using diagnostic tools and assessments, enabling a tailored approach to each patient's care.
• Provide hands-on treatments, including manual therapy and therapeutic exercises, while instructing patients on daily living activities and self-management strategies.
• Assess and make modifications to the home environment to support the patient's rehabilitation process, enhancing their level of independence.
• Monitor patient progress and document changes in their condition, ensuring that the treatment plan is consistently updated and communicated to the multidisciplinary team.
• Collaborate with Physical Therapy Assistants, providing guidance and oversight to ensure their compliance with the treatment protocols.
• Maintain accurate clinical documentation, including initial evaluations and progress notes, adhering to regulatory requirements and organizational policies.
• Engage in regular team meetings to discuss patient cases, share insights, and strategize on holistic treatment approaches.
• Educate patients and their families on the appropriate use of therapy equipment and empower them with knowledge to enhance their participation in the therapy process.
• Stay current with professional development and best practices in physical therapy to continuously improve your skills and the quality of care provided.

Patient Population / Client Focus

The successful candidate will work primarily with a diverse patient population, including elderly clients, individuals with chronic illnesses, post-surgical patients, and those needing rehabilitation after injury or illness. Our approach is uniquely centered around each client's needs, with an emphasis on building strong, trust-based relationships that foster optimal recovery outcomes.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required; a Doctorate in Physical Therapy is preferred.
• Current licensure as a Physical Therapist in the state of New Jersey, with a clean record of standing.
• Certification in CPR and a commitment to maintaining up-to-date knowledge in the field of physical therapy.
• A minimum of one year of hands-on experience as a Physical Therapist, particularly in a home care or acute care environment.
• Demonstrated ability to work independently while effectively collaborating with team members.
• Strong communication skills to engage effectively with clients, families, and other healthcare professionals.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 – 120,000 per year, along with a comprehensive benefits package that includes health insurance, retirement plans, paid time off, and continuing education support.
