Per Diem Licensed Practical Nurse (LPN)
Job Summary:
Champion City Nursing & Rehabilitation, a leading provider of high-quality healthcare services, is seeking a skilled and compassionate Per Diem Licensed Practical Nurse (LPN) to join our dynamic team! As a Per Diem LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and residents while working in a fast-paced and supportive environment.
Responsibilities:
• Provide direct patient care, including administering medications, wound care, and vital signs, under the supervision of a registered nurse
• Develop and implement individualized care plans to meet the unique needs of each patient
• Collaborate with multidisciplinary teams, including physicians, therapists, and other healthcare professionals to ensure comprehensive care
• Participate in patient education and discharge planning to promote optimal outcomes
• Maintain accurate and detailed records of patient interactions, treatments, and outcomes
Requirements:
• Graduate of an accredited LPN program
• Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania
• Strong clinical and communication skills
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
• Strong knowledge of medical terminology, anatomy, and physiology
