Housing Stability Case Manager

CP Unlimited is seeking a Case Manager to support residents in achieving housing permanency through empathetic, goal-driven case management. You will build collaborative relationships, provide information, referrals, and coordinate services such as health care, job training, housing, and immigration resources. Responsibilities include managing a caseload of 25 clients, conducting intakes within 24 hours, developing individualized service plans, monitoring progress, and advocating with community #J-18808-Ljbffr
