Job Description
Physical Therapist Job Overview We are seeking a dedicated Physical Therapist to join our Home Health team. This position involves providing quality care to patients in their homes, ensuring the highest standards of rehabilitation and therapy services. Key Responsibilities
- Conduct patient evaluations and develop individualized treatment plans.
- Perform home health visits, including initial assessments (SOC), recertifications (ROC), and ongoing evaluations.
- Treat a variety of patients with diagnoses including congestive heart failure (CHF), orthopedic conditions, and pneumonia.
- Manage an average caseload of approximately 6.5 patients per day.
- Collaborate with the clinical team and adhere to established care protocols.
- Participate in staff education and development initiatives.
- Minimum of 1 year of experience in physical therapy.
- Current Basic Life Support (BLS) certification is required.
- Prior experience in home health care is preferred.
- No new graduates accepted; experience preferred.
- Dress Code: Scrubs or business casual attire.
- Provided with necessary technology including a phone and tablet for patient management.
- Accessible support from a Clinical Manager or Educator during working hours.
