Bilingual Licensed Vocational Nurse

Bilingual LVN - Primary Care / FQHC

Monday-Friday | 8:00 AM-5:00 PM (1-hour lunch)
$35-38/hr

We are seeking a compassionate and adaptable Spanish-speaking LVN to join a busy outpatient primary care/FQHC setting in San Fernando. This role offers a balanced mix of clinical and administrative responsibilities while working closely alongside an experienced RN and provider team.


Responsibilities

• Provide direct patient care in an outpatient primary care environment
• Assist with prescription refills, prior authorizations, and patient triage
• Support chronic disease management for patients with:
  • Type 2 diabetes
  • Hypertension
  • Obesity
  • Dietary and lifestyle concerns
• Educate patients on medications, glucose monitoring, blood pressure tracking, and overall wellness
• Conduct routine follow-up calls and patient check-ins
• Collaborate closely with the RN and provider team to coordinate patient care
• Assist with injections and blood draws as needed
• Maintain accurate documentation and support daily clinic operations
• Work with a wide variety of patient populations and complex cases

Qualifications


Required

• Active LVN license
• BLS/CPR certification
• Fluent Spanish bilingual skills
• Comfortable in a fast-paced outpatient setting
• Experience with patient education and chronic care management

Preferred

• 2+ years of LVN experience
• Hospital or primary care/FQHC background
• Pharmacy experience, including prescription refills and prior authorizations
• Injection and venipuncture experience
• IV certification preferred but not required
