Our mission is to provide exceptional and compassionate homecare services to patients who need our help the most.We are seeking talented, passionate individuals to join our team as a Patient Care Manager and help our patients live happier and healthier lives.What We Offer:Make a difference every day in the lives of those who need our help the mostCompetitive payPaid on a weekly basisMedical/dental/vision/life insurancePaid holidays/PTO/401(k) matchCareer growth opportunitiesGreat and collaborative work environmentWork-life balanceResponsibilitiesConduct home assessments to determine consumer eligibility for Home Care servicesEffectively communicate with Direct Care Workers and patients about company updates regarding their services.To supervise, observe and monitor contact visits.To advise, guide, and train household members in managing household affairs', practical tasks, and other means of providing for care.Complete routine updates on the well-being of patients and direct care workers.Lead and supervise a team of Care Team Associates (CTA)Ensure that patients receive personal care services based on needs and what matters most to the patientMeet operational and growth goals, including key performance metricsEnsure quality of services delivered meets Company standardsFoster an engaged workforceEnsure compliance with the law and Company policiesAssist CTAs with their day-to-day duties, including screening new patients and caregivers, on-boarding new patients and caregivers, staffing patient cases with caregivers and ensuring patient cases are started promptly, communicating with patients, caregivers, and patient families to ensure satisfaction and quality service delivery, and assist with on-site visits as neededQualificationsPrevious experience in home care, nursing or other medical fieldsAbility to build rapport with patientsAssociate's degree or Bachelor's degree requiredTwo or more years of healthcare leadership preferredBilingual preferredPassion and dedication to help those in needStrong work ethicStrong communication skillsAbility to work flexible hours to meet patient and employee needs
