We are seeking a dedicated Registered Nurse (RN) with strong home health experience to join a patient-focused team providing care in a field-based environment.Schedule:Full-time, 13-week contractMonday Friday, 8:00 AM 5:00 PMManage a caseload of 2532 points weekly (typically 35 patient visits per day)Daily travel within assigned territory (mileage reimbursement provided)Responsibilities:Complete comprehensive assessments using OASIS and develop individualized care plans.Deliver skilled nursing services including wound care, medication management, IV therapy, and chronic disease education.Monitor patient status, identify changes in condition, and communicate developments to physicians and the interdisciplinary team.Provide thorough patient and family education to promote health management and safety.Ensure timely, accurate documentation with Home Care Home Base (HCHB).Collaborate with physical and occupational therapists, home health aides, and medical social workers to support optimal patient outcomes.Attend case conferences, staff meetings, and essential ongoing training.Maintain compliance with regulatory and agency standards.Qualifications:Active RN license (in good standing)At least 1 year of recent field-based Home Health RN experienceProficiency in Home Care Home Base (HCHB) and OASIS documentationExceptional clinical assessment, organizational, and communication abilitiesValid drivers license, reliable transportation, and auto insuranceBSN and experience managing 2532 points per week preferredUnderstanding of Medicare and home health regulations a plusBenefits:Weekly pay scheduleComprehensive medical, dental, and vision insurance options401(k) retirement plan optionsMileage reimbursement for work travelOngoing 24/7 supportOpportunity for contract extension or renewalApply today to make a meaningful difference in patients lives while advancing your nursing career in a supportive, flexible, and professionally rewarding environment!$1,897 - $2,007/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates.
