Continuum Therapy Partners is hiring a licensed Physical Therapist (PT) for a full-time position providing physical therapy services at Taylorville Care Center in Taylorville, IL and Montgomery Nursing and Rehab Center in Hillsboro, IL .

Physical Therapist (PT) Responsibilities

• Evaluate and treat adult and geriatric patients in a skilled nursing facility.
• Develop individualized physical therapy treatment plans.
• Improve patient mobility, strength, balance, endurance, transfers, gait, and functional independence.
• Perform physical therapy evaluations and reassessments.
• Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and rehabilitation staff.
• Maintain timely and accurate documentation.
• Educate patients and caregivers on home exercise programs, mobility, fall prevention, and safety.
• Deliver compassionate, evidence-based physical therapy services focused on quality patient outcomes.

Physical Therapist (PT) Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Illinois Physical Therapist (PT) license.
• Skilled Nursing Facility (SNF), long-term care, rehabilitation, or outpatient experience is preferred but not required.
• New graduate Physical Therapists are encouraged to apply.
• Excellent communication and teamwork skills.
• Ability to travel between Taylorville and Hillsboro locations.
