Patient Navigator

Role ImpactThe Patient Navigator is an outcome-focused professional who ensures our patients have an effective and efficient experience. You will act as the "connective tissue" of the CCBHC network, working collaboratively with clients, families, and external providers to close referral gaps and ensure no one falls through the cracks.Key ResponsibilitiesData & Reporting: Collect and manage data for CCBHC outcomes, including National Outcome Measures (NOMs) and other required reporting.Clinical Monitoring: Track external data to identify hospital readmissions for psychiatric or substance use within the previous 30 days.Safety Coordination: Responsible for the monitoring and necessary follow-up of patients on the Zero Suicide pathway.Patient Advocacy: Build therapeutic rapport using trauma-informed care; ensure patients have all required information for their "next steps" before leaving their appointments.Collaboration: Facilitate communication between counselors, support staff, and community resources (teachers, pastors, coaches, etc.).Certification: Must hold/maintain Kansas Case Manager Certification.Education: Minimum education required; experience in behavioral health preferred.Requirements: At least 18 years old with a valid Kansas Driver’s License and proof of insurance (for patient transport).Screening: Must pass KDADS, Child, and Adult background checks.Skills: Strong keyboarding, basic math, and the ability to react effectively in diverse human service situations.Benefits for FULL TIME Position:Affordable Blue Cross Blue Shield health insuranceRetirement Plan (401k); match after 1 year of employmentGenerous Paid Time Off (PTO) accrualsCompany paid life and disability insuranceEmployee Assistance ProgramDelta Dental of KansasVision DirectFlexible Spending AccountHealth Savings Account with employer contributionBereavement LeavePlus much more #J-18808-Ljbffr
