We're building a company culture that embraces the values of respect, accountability, communication, teamwork, integrity, positive attitude, continuous improvement and employee appreciation.What You Will Do:Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC.Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care.Autonomy within your position, use your training, critical thinking and knowledge of your patients to make decisionsSubmits clear and appropriate documentation in a timely manner and documents patient education.Communicates effectively to physicians, supervisors, and co-workers in a timely manner.Promotes continuity of care with appropriate admissions, transfers, and charges.Other duties as assigned.Skills:Effective communication skills (verbal and written)Experience with Electronic Medical Records (EMR) software is preferredTime management and Organizational skillsDemonstrated ability to work autonomously as a registered nurseExcellent planning, organizing and communicating.What you will need:Graduate of an accredited school of professional nursing, BSN preferred, currently licensed as an RN through the Ohio State Board of Nursing.Prior Nursing experience in Hospice or Home Health setting preferred. At least one year's medical/surgical nursing experience preferred.BLS/CPR Certification requiredMust possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy.
