Therapist - Physical Therapist

Job Summary:

We are seeking a dedicated Physical Therapist to provide rehabilitation services to patients in a skilled nursing facility setting. The role involves developing and implementing personalized treatment plans to improve patient mobility and function.

Responsibilities:

• Assess patients’ physical conditions and establish treatment goals.

• Develop and modify individualized therapy plans based on patient progress.

• Utilize therapeutic exercises and techniques to restore mobility and strength.

• Document patient assessments, treatment interventions, and progress notes accurately.

• Collaborate with healthcare team members to ensure comprehensive patient care.

• Educate patients and families on therapeutic exercises and health maintenance.

Qualifications:

• Valid licensure as a Physical Therapist in the state.

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy.

• Proven experience working with geriatric populations in SNF settings preferred.

• Strong clinical assessment and treatment plan development skills.

• Excellent communication and interpersonal abilities.

• Ability to multitask and manage a diverse caseload.
