Occupational Therapist Home Health

CenterWell Home Health is seeking an Occupational Therapist for in-home care to help patients regain independence, improve safety, and enhance quality of life in the comfort of their own homes. As part of the Healthcare & Medical Services team, you will provide skilled occupational therapy interventions, focusing on functional mobility, activities of daily living, home safety, and adaptive equipment training. You will complete comprehensive in-home evaluations, develop individualized treatment plans, and deliver evidence-based therapeutic interventions tailored to each patient’s goals and medical needs. Working closely with nurses, physical therapists, speech therapists, physicians, and caregivers, you will coordinate care to ensure holistic, patient-centered treatment. Daily responsibilities include conducting home safety assessments, recommending and training patients on assistive devices and durable medical equipment, educating patients and family members on safe techniques and energy conservation, and documenting all care in accordance with regulatory and CenterWell standards. You will monitor patient progress, adjust care plans as needed, and communicate changes in condition promptly to the interdisciplinary team.
