You will manage a field-based caseload, traveling between patient homes during your shift and tailoring each treatment plan to the unique layout and needs of each home.Typical responsibilities include completing comprehensive PT evaluations in the home, developing individualized plans of care, and providing evidence-based interventions focused on mobility, strength, balance, and functional independence. You will educate patients and caregivers on safe transfers, use of assistive devices, home exercise programs, and fall prevention strategies that are directly applicable to daily life in their own environment. Coordination with nursing, occupational therapy, speech therapy, and other disciplines will be an important part of ensuring safe, effective, and integrated care.You can expect to work with primarily adult and geriatric patients with a variety of diagnoses, including post-acute, post-surgical, orthopedic, neurological, and cardiopulmonary conditions. Visit volumes and schedules are designed to support quality of care and thorough documentation rather than rushed encounters. Shifts are typically daytime hours, with some flexibility in scheduling to accommodate patient needs and optimize your travel route across the Taylor region.The work environment emphasizes professional growth, clinical quality, and a supportive culture for clinicians practicing out in the field.
