CSI is seeking a dedicated and passionate Case Manager to join our team at our new Multi-Dimensional Anti-Recidivism Partnership (MAP) located in West Valley, CA. Adult MAP is an innovative program dedicated to engaging the community and providing rehabilitative programming for justice-involved individuals. We focus on promoting long-term, self-sustaining skills through vocational training and personal development services. This model hosts a variety of sub-contractors at one site to reduce barriers for clients. As a Case Manager at our MAP programs, you will be working closely with the Program Director, providing re-entry services to a criminal justice impacted population. You will be responsible for computerized facility information and will complete reports as needed/directed. What You’ll Do Perform client needs assessments within seven (7) days of intake and enter data into CSI data systems on the day the data is obtained. Enter into funder database within five (5) days of client advancement into Services. Actively participate in Client Progress Review (CPR) meetings by presenting new clients, assessments and Individual Service Plan updates, as well as discharge planning for clients nearing discharge. Provide and elicit peer support for client issues. Develop and document program plan goals with clients within thirty (30) days of activation and review with client on a bi-weekly basis. Enter goals and relevant client data into Individual Service Case Plan and ensure ARMS database reflects case planning efforts and real-time progress. Monitor program stipulations, job readiness preparation, and community resource referrals. Case Managers are responsible for weekly curricula facilitation including: entering all new referrals to interventions they facilitate into the CSI data systems on the day of the referral; preparing, planning and facilitating individual and group sessions with integrity and fidelity to curriculum/manual; scheduling and conducting make-up sessions as needed; documenting group and individual sessions in the agency data systems on the same day the information is received as well as in the funder database; maintaining a minimum of 75% completion rate for each intervention facilitated; and abiding by the Agency’s programs, policies, procedures and code of ethics. Perform other tasks as assigned by supervisor. Qualifications Bachelor’s degree and one year experience in case management or similar field. Education may be substituted with experience on a year-for-year basis. Strong decision-making, organizational, and interpersonal skills. Previous assessment and facilitation experience preferred. Experience using Microsoft Office and a commitment to working as part of a team.
