We are seeking an experienced and compassionate Part-Time PRN Hospice RN Case Manager to provide exceptional, patient-centered end-of-life care throughout the Dallas–Fort Worth area. As a PRN RN Case Manager, you will oversee the care of assigned hospice patients, conduct comprehensive assessments, coordinate services, manage symptoms, and support patients and families throughout the hospice journey. You will collaborate closely with physicians and the interdisciplinary hospice team to promote comfort, dignity, and quality of life.

Service Areas
Local travel is required. Patient assignments will be based on the nurse’s availability, geographic location, patient census, and agency needs.

Key Responsibilities

Patient Assessment and Care Planning

• Conduct comprehensive initial and ongoing nursing assessments for assigned hospice patients.
• Develop, implement, and update individualized Plans of Care based on each patient’s condition, goals, needs, and end-of-life preferences.
• Monitor patients for changes in condition and promptly revise or recommend updates to the Plan of Care when appropriate.
• Identify changes in pain, symptoms, functional ability, safety, caregiver support, and overall care needs.
• Communicate significant changes or concerns to the physician, Medical Director, clinical leader, and interdisciplinary team.

Skilled Nursing and Symptom Management

• Provide skilled nursing care and comfort-focused interventions in accordance with the patient’s Plan of Care.
• Assess and manage pain, respiratory distress, anxiety, agitation, nausea, wounds, and other end-of-life symptoms.
• Evaluate the effectiveness of medications and interventions and coordinate necessary changes with the appropriate provider.
• Respond appropriately to urgent clinical concerns and help prevent avoidable hospitalizations when consistent with the patient’s goals of care.

Care Coordination

• Coordinate patient care with physicians, the Medical Director, social workers, chaplains, hospice aides, facility staff, pharmacies, medical equipment providers, and other interdisciplinary team members.
• Help ensure that medications, supplies, medical equipment, and hospice services are available as ordered.
• Maintain consistent communication with patients, caregivers, facilities, and members of the interdisciplinary team.
• Promote continuity of care by providing clear and timely updates regarding patient status and care needs.

Patient and Family Education

• Educate patients and caregivers about disease progression, medications, symptom management, comfort measures, medical equipment, safety, and caregiving techniques.
• Reinforce when and how to contact hospice for questions, changes in condition, or urgent concerns.
• Provide compassionate guidance and emotional support to patients and families throughout the hospice journey.

Documentation and Compliance

• Complete all assessments, visit notes, care coordination records, and other required documentation accurately and within agency-established timeframes.
• Ensure clinical notes clearly describe the patient’s condition, interventions provided, response to care, caregiver concerns, and follow-up plan.
• Maintain documentation that supports continuity of care, hospice compliance, and the patient’s continued eligibility for hospice services.
• Follow all hospice regulations, professional nursing standards, HIPAA requirements, and Millenia Hospice policies and procedures.

Interdisciplinary Team Participation

• Collaborate with all members of the interdisciplinary hospice team.
• Participate in Interdisciplinary Group meetings as assigned or required.
• Provide meaningful clinical updates regarding assigned patients.
• Recommend changes to the Plan of Care based on patient and family needs.
• Participate in the on-call rotation as needed on a PRN basis.

Required Qualifications

• Current and unrestricted Registered Nurse license in the State of Texas.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability and willingness to travel throughout the assigned service area.
• Strong clinical assessment, case-management, care-coordination, and critical-thinking skills.
• Excellent communication, organizational, and interpersonal abilities.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Ability to manage time, prioritize changing patient needs, and complete assigned visits.
• Ability to maintain accurate, complete, and timely clinical documentation.
• Ability to successfully complete required background checks, license verification, and onboarding requirements.
• Commitment to maintaining confidentiality and complying with HIPAA and agency policies.

Preferred Experience

• One to two years of hospice, home health, palliative care, or related nursing experience is preferred.
• Previous RN case-management experience is highly valued.
• Experience with pain and symptom management, end-of-life care, and patient and caregiver education is preferred.
