Key Responsibilities of the Physical Therapist

Perform physical therapy evaluations and treatments based on physician orders/referrals.
Complete comprehensive evaluations using suitable evaluation and assessment tools and procedures.
Review patient records for current diagnosis, medical history, precautions, and contraindications.
Establish measurable goals and develop treatment plans for patient independence and function improvement.
Reassess treatment plan effectiveness, patient progress, and modify as necessary.
Communicate patient needs and progress to the treatment team, physician, and relevant parties.
Consult with medical providers on patient evaluation, treatment, and discharge.
Recommend and assist with securing adaptive equipment to improve patient function.
Maintain and submit all required documentation in compliance with regulations.
Supervise Physical Therapist Assistants and Aides according to state practice act.
Participate in staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist

Current PT license
Home Health (HH) experience
Experience with HCHB (Home Care Home Base)
Ability to cover Mesa County and Garfield County
