Independent Living Skills Case Manager

About the Role – Responsibilities Provide one‑on‑one individualized life skill coaching to develop essential skills and promote independent living. Collaborate with individuals, service teams, and supports to create goals, strategies, and tools for personal growth toward independence. Partner with people served to develop and implement individualized support plans, setting goals and tracking progress. Promote healthy lifestyle choices through exercise programs, nutrition guidance, and wellness activities. Support adults with disabilities in engaging in recreational activities that encourage socialization, teamwork, and personal growth. Assist with job skill training to explore career interests and pre‑vocational skills. Document learning and goal progress in the case management system. Education and Work Experience Applicants are encouraged to apply even if they do not meet all listed qualifications. 1+ year of previous work or volunteer experience supporting adults with disabilities Experience in professional settings and work environments Associate degree or working toward completion Hiring Requirements Proof of negative TB test results Verification of occupational physical clearance Pass a criminal fingerprint clearance 18 years of age or valid job permit Physical Activity Physical stamina to engage in domestic, job training, and community activities Ability to drive vehicle and transport adults with disabilities Lift up to 25 lbs. as needed Equal Employment Opportunity AbilityPath is an equal opportunity employer.
