Discharge Planner Job Summary:
We are seeking a skilled and compassionate Discharge Planner to join our interdisciplinary team. As a Discharge Planner, you will be responsible for coordinating the transition of our patients from our facility to their next level of care, ensuring a seamless and efficient discharge process. Discharge Planner Responsibilities:
Coordinate with multidisciplinary teams, including physicians, nurses, and social workers, to develop comprehensive discharge plans for patients
Conduct patient assessments and identify barriers to discharge, developing strategies to overcome these barriers
Facilitate communication between patients, families, and healthcare providers to ensure a smooth transition to the next level of care
Monitor patient progress, adjusting discharge plans as needed to ensure optimal outcomes
Identify and address potential discharge delays, implementing corrective actions to maintain efficient discharge processes
Maintain accurate and timely documentation of discharge plans, patient progress, and coordination efforts
Requirements:
Prior experience in discharge planning or a related field preferred
Degree in Social Services or related field a plus
Strong communication, organizational, and problem-solving skills
Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines
Proficiency in electronic medical records and discharge planning software
Strong attention to detail, with excellent documentation and record-keeping skills
Ability to maintain confidentiality and handle sensitive information with discretion
