Job Requirements

• Active Registered Dietitian credential (RD or RDN) required
• Minimum 1 year of experience as a Registered Dietitian; dietary or long-term care (LTC) experience preferred
• Experience with clinical nutrition, nutrition assessments, and developing nutrition care plans
• Strong communication, documentation, and organizational skills; comfort working with interdisciplinary teams
• Ability to work on-site in West Reading, PA and adapt to facility policies and EHR systems

Responsibilities

• Conduct nutrition assessments, develop and implement individualized nutrition care plans for patients in dietary and long-term care settings
• Collaborate with physicians, nurses, allied health professionals, and food service staff to optimize clinical nutrition outcomes
• Provide patient and family education on therapeutic diets, nutrition interventions, and dietary modifications
• Monitor, evaluate, and document patient progress; adjust care plans as needed to meet clinical goals
• Ensure compliance with regulatory standards, facility policies, and best practices in dietary services
• Participate in quality improvement, nutrition screenings, and interdisciplinary care planning
