Physical Therapist

Life Care Centers is seeking a compassionate and dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide patient-centered rehabilitative care to primarily senior and adult patients within a post-acute and long-term care environment. Working within Life Care Centers’ family-focused culture, you will evaluate each patient’s functional status, develop individualized treatment plans, and deliver evidence-based physical therapy interventions that promote mobility, safety, and independence. You will collaborate closely with an interdisciplinary team—including nursing, occupational and speech therapy, and medical providers—to ensure coordinated, high-quality care and strong clinical outcomes. Day-to-day, you will complete thorough assessments; design and implement therapeutic exercises, gait and balance training, and functional mobility programs; and educate patients and families on safe movement strategies, home exercise programs, and equipment use. You will document all evaluations, progress notes, and treatment outcomes in accordance with regulatory and company standards, maintaining accurate and timely electronic medical records. The Physical Therapist will actively contribute to discharge planning, recommend appropriate assistive devices, and participate in quality improvement initiatives that support Life Care Centers’ commitment to excellence.
