Physical Therapy - PT SNF
This position is for a Physical Therapist (PT) working Monday–Friday from 8:00 AM – 4:30 PM, with 40 hours per week and no weekend rotation. This is a permanent placement opportunity in a Skilled Nursing Facility (SNF), offering a salary range of $49–$55/hour. The PT provides rehabilitation services to patients recovering from illness, injury, or surgery, focusing on restoring mobility and functional independence.
Requirements: Candidates must hold an active Physical Therapist license and have graduated from an accredited Physical Therapy program (DPT or PT). Experience in SNF, long-term care, or rehabilitation settings is preferred. Strong skills in patient evaluation, treatment planning, and EMR documentation are required.
The PT will perform comprehensive evaluations, develop individualized care plans, and deliver therapy interventions including gait training, strengthening, balance training, and mobility exercises. Responsibilities include monitoring patient progress, modifying treatment plans, documenting sessions, and collaborating with interdisciplinary teams to ensure safe and effective patient outcomes.
