BENEFITS Immediate Paid Time Off (PTO) accrual Competitive wages and shift differentials Medical, Dental, and Vision insurance (available within 30 days) 401(k) retirement plan with company match Tuition assistance RESPONSIBILITIES Develops and completes resident assessments, including timely completion, transmission, and updates; implements RAPs and triggers; evaluates condition, medical background, and special assessment/activities of each resident; coordinates interdisciplinary meetings. Assists with plans of care, ensuring timely completion of comprehensive care plans, conducting quarterly or regular reviews, and coordinating review by interdisciplinary team. Assists with payer coverage matters, including pre‑admission assessment, Medicare and payer coverage coordination, clinical financial analysis for the RAI process, coding assistance, and ensuring documentation supports covered services per Medicare guidelines. Act as chairperson of the Interdisciplinary Care Plan Team, developing comprehensive resident assessments and care plans, providing training on the RAI process, and coordinating documentation to support MDS. Ensures proper documentation is maintained, verifying each assessment portion is signed and dated by the completing individual. Performs quality assessment and assurance functions, including ongoing review of quality, auditing the RAI process, assisting with audits, and communicating results to staff. Assists in developing and updating assessment and care plan policies and procedures to reflect facility philosophy, professional standards, and legal requirements. Participates with Executive Director or Director of Nursing/designee in survey processes, instructing staff, reporting, and directing corrective action when needed. Maintains and applies current skills through continuing education and in‑service programs. Carries out responsibilities in compliance with federal, state, and local laws, facility philosophy, policies, and procedures. Has the ability to read and interpret technical journals, financial reports, and legal documents; respond to inquiries or complaints from residents, regulatory agencies, or community members; and present information to management. Demonstrates excellent interpersonal skills, high energy, enthusiasm, organization, and strong verbal and written communication skills. Position requires standing, sitting, stooping, pushing, pulling, lifting up to 75 pounds or more, bending, twisting, walking, running, carrying, listening, talking, use of telephone, and exposure to medical equipment and individuals at risk of illness. SUPERVISORY RESPONSIBILITIES None. JOB REQUIREMENTS Education and/or experience: Completion of a registered nursing program. Experience with MDS, resident assessments, care plans, and RAPS preferred.
