Adult Health Home Care ManagerOnondaga County, NY | Hybrid Salary: $50,000 - $52,000 Liberty Resources is seeking an Adult Health Home Care Manager to support adults with medical, behavioral health, and social service needs through care coordination and community-based support.Care Manager Responsibilities:Assess needs and develop person-centered care plansCoordinate services with healthcare and community providersConnect members to medical, mental health, housing, and social service resourcesConduct home and community visitsManage a caseload of approximately 30 individualsParticipate in an on-call rotationCare Manager Qualifications:Bachelor's degree + 2 years of relevant human services experience; or Master's degree + 1 year of relevant experience; or RN with 2 years of relevant experienceValid NYS driver's license and reliable transportationCare management/case management experience preferredExperience working with adults with mental health, substance use, developmental disabilities, or chronic health needs preferredBenefits:Medical, Dental & Vision InsuranceGenerous Paid Time Off401(k)Continuing Education OpportunitiesSupportive, team-oriented work environmentLiberty Resources is an Equal Opportunity Employer.
