Contract Term: March 1, 2026 – February 28, 2027 (1 Year)

Work Arrangement: Onsite – Weekly presence required, plus quarterly meetings

To appoint a qualified and licensed physician to serve as the Medical Director for Shady Lane Nursing Home, providing medical leadership and oversight of resident care. The Medical Director will ensure clinical excellence, regulatory compliance, and active participation in quality assurance, pharmacy, and governance programs to promote optimal care outcomes for all residents. The Medical Director provides clinical oversight, ensures regulatory compliance, and supports operational quality at Shady Lane Nursing Home. Responsibilities include developing medical policies, attending required governance meetings, overseeing care documentation for Medicare and Medicaid residents, and collaborating closely with facility leadership to maintain high-quality resident outcomes.

Key Responsibilities

1. Medical Leadership & Oversight

• Provide medical leadership and coordinate all resident medical care.
• Develop, implement, and maintain clinical policies and procedures manuals.
• Support medical staff and ensure compliance with federal, state, and local regulations.
• Collaborate weekly with the Director of Nursing on care issues, survey preparation, and IDR (Informal Dispute Resolution) compliance.

2. Governance & Quality Assurance

• Attend required meetings:
  • Quarterly QAPI (Quality Assurance and Performance Improvement) Meetings
  • Quarterly Pharmacy & Therapeutics (P&T) Meetings – Held on the 3rd Wednesday at 1:30 PM (January, April, July, October).
• Participate in governance discussions, policy updates, and clinical improvement initiatives.
• Support survey readiness and audit preparedness throughout the year.

3. Documentation & Regulatory Compliance

• Ensure monthly documentation for all Medicaid residents.
• Ensure weekly documentation for all Medicare Part A residents.
• Utilize Sigma Care System for order writing, documentation, and chart updates.
• Maintain up-to-date knowledge of Medicare/Medicaid requirements and clinical documentation standards.

4. Administrative & Billing Requirements

• Maintain an active New Jersey Physician License, CDS License, and Malpractice Insurance (minimum $1M coverage).
• Designate a qualified, licensed professional for on-call coverage during absences, with written notice to the facility.
• Bill/invoice directly to residents, Medicare, Medicaid, managed care, or other third-party payers per federal and state laws.
• Ensure all credentials, licenses, and insurance remain current throughout the contract term.

Education, Experience & Qualifications

• Licensed Physician in the State of New Jersey – Mandatory
• Controlled Dangerous Substances (CDS) License for Shady Lane – Mandatory
• Active Malpractice Insurance (Minimum $1,000,000 coverage, valid throughout contract) – Required
• Experience with Long-Term Care Residents (Medicare/Medicaid documentation) – Required
• Familiarity with Sigma Care System or similar EHR platform – Preferred
• Proven ability to manage regulatory compliance, policy implementation, and interdisciplinary leadership in a healthcare setting.
