What You Bring:

• Active RN license in D.C.

• 2 years of nursing experience (Home Health or Home Care preferred)

• Reliable transportation and valid drivers license

• Strong communication documentation and assessment skills

• A commitment to compassionate high-quality care

• Must be tech-savvy

What Youll Do:

• Travel for home visits and caregiver supervision

• Conduct in-home patient assessments

• Create and manage individualized care plans

• Monitor patient progress and adjust care as needed

• Educate patients and families on health and wellness

• Collaborate with a professional care team dedicated to improving lives
