Key Responsibilities

• Provide physical therapy services in an inpatient setting as the primary area of focus
• Conduct home health visits as part of a combined caseloadutilize company-provided vehicles for home health patient visits
• Document patient care accurately using multiple electronic medical record systems
• Develop and implement individualized treatment plans to support patient recovery and functional goals

Required Qualifications

• Active Physical Therapist license in good standing
• Experience working in inpatient and/or home health settings
• Proficiency with electronic medical record platforms including EPIC, Net Health, PCC, and/or Maestro
• Valid driver's license and ability to operate a company vehicle for home health visits

Preferred Experience / Skills

• Prior experience in both inpatient rehabilitation and home health environments
• Familiarity with multiple EMR systems used across care settings
