- Collaborate with a multidisciplinary team of healthcare professionals to provide comprehensive behavioral health services to patients in long-term care and rehabilitation centers
- Conduct thorough assessments, diagnoses, and treatment planning for patients with acute and chronic mental health conditions
- Develop and implement individualized treatment plans to promote patient recovery and well-being
- Provide evidence-based psychotherapies and medication management to patients
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes
- Stay current with industry developments, best practices, and evidence-based treatment approaches to continuously improve patient care and outcomes
