Hospice Registered Nurse Case Manager $5,000 Sign On Bonus

Role We are looking for a Registered Nurse Case Manager to support hospice care patients and families in Eaton County. Responsibilities Ensuring that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team. Assessing hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations. Obtaining data on physical, psychological, social, and spiritual factors that may influence patient or family health status. Communicating significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care. Counseling, instructing, and involving the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaching hospice philosophy and pain and symptom management to staff in alternate care settings such as nursing homes, hospitals, and assisted‑living facilities. Providing support at the time of death and performing bereavement assessment. Qualifications Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing. Current State License as a Registered Nurse (RN). Minimum one year of experience in a clinical care setting. Experience in a hospice or home health environment is preferred. 50% travel required. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. Valid driver’s license, insurance, and reliable transportation. Benefits Competitive compensation package. Sign‑on bonus of $5,000.
