Description

• Conduct evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their unique needs.
• Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
• Work collaboratively with a multidisciplinary team, including nurses, physicians, case managers, and rehabilitative specialists, to optimize patient outcomes.
• Educate patients and their families on therapy goals, discharge planning, and home exercises to foster recovery.
• Maintain accurate documentation of evaluations, treatments, and patient progress in alignment with facility protocols and regulatory standards.
• Engage in interdisciplinary discussions during care team rounds to ensure seamless patient care.

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited program.

Certifications

• Valid state licensure as a Physical Therapist.
• Preferred: CPR certification.
• Additional certifications in specialized areas (e.g., neurological, geriatric) are advantageous but not mandatory.
