Job Description The Case Manager works under the direction of the Executive Director. The Case Manager assists during the admission and discharge process of residents and coordinates meetings with residents during their stay to evaluate and effectively contribute to an individualized plan of care and discharge goals. The Case Manager works closely with fellow IDT members to manage skilled services and ensure documentation is in place to support skilled coverage.
