Description

• Conduct assessments of patients in both inpatient and outpatient contexts, crafting tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, building strength, and improving overall functionality
• Work collaboratively with nurses, physicians, case managers, and other therapy professionals to achieve optimal outcomes for patients
• Guide patients and their families through therapeutic objectives, discharge preparations, and at-home exercise regimens
• Keep thorough documentation of evaluations, treatments, and patient progress, adhering to regulatory guidelines
• Engage in care team discussions and interdisciplinary meetings to ensure seamless care coordination

Education

• Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., geriatric, neurological) are advantageous but not mandatory

Skills

• Prior experience in a clinical setting is strongly preferred
